If you’re thinking of visiting Vietnam for a holiday or business, you need to make sure that you have the right visa to be allowed entry to the country. When applying for a Vietnam Visa, one of the steps is to fill out the NA1 form, also known as the Visa Application Form. It’s important that you fill out this form completely and that you provide accurate information on the form. This form will be submitted along with your passport at the Visa landing counter in the airport of Vietnam for your Vietnam Visa on Arrival to be approved.

What to Submit Along With the Form

The Visa Application Form is not the only requirement that you need to submit for your Vietnam Visa. Here are the other requirements that you need to provide along with the form:

How To Fill Out the NA1 Form?

Remember that the approval of your Vietnam Visa on Arrival will be based on the information that you provided on your form. If you don’t fill out the form correctly or provide incorrect information, you may not be approved of the visa and you can’t be allowed entry to Vietnam. To avoid this problem, we’ll provide the proper steps of how to fill out the form.

1. First, attach your photo on the form. Make sure your photo has the correct size, which is either 3 x 4 cm, 4 x 6 cm, or 5 x 5 cm and the background should be white. Before you leave for Vietnam, make sure you have your photo taken and attach it on the completely filled out form.
2. Next, provide your complete name. Make sure you write down your full name in capital letters. This should be the same name that appeared in your passport. Both first name and last name must be written on the form, along with your middle name if you have.
3. Next, indicate what your gender is. Check the box that corresponds to your sex.
4. Indicate your date of birth. It should be in this format – DD / MM / YY.
5. You must also provide your place of birth. This information will be provided in your passport so you need to have the same information on the form. Double-check your passport if you’re not sure.
6. Next, provide your nationality at birth. You also need to indicate your current nationality. This should be the same as what’s indicated in your passport.
7. Next, write down your religion. If you don’t follow any religion, then leave this blank.
8. You will also be asked to indicate your current work. Along with this, you need to indicate the name of your employer as well and the address of your work. If you’re still a student, you need to indicate the name of your school.
9. Next, indicate your permanent address and include your phone number and email address. Make sure you indicate your complete residential address. This should be the address where you are currently living.
10. Next, you will need to provide information about your family members. This will include the complete name of your father, mother, or any members of your family that is living in your country and not together with you on the trip. The information will be used in the event of an emergency.
11. The next field on the form will come from your passport. This includes your passport number, type of passport, issuing authority, place of issue, and expiry date of your passport. Refer to your passport for these details to make sure that the information you will provide is correct.
12. If you have been to Vietnam before, you need to indicate the date of your visit. For those who are visiting Vietnam for the first time, then leave this field blank.
13. Next, indicate your intended date of entry to Vietnam. You need to also provide information as to how long you plan on staying in the country.
14. You will also be asked on what your purpose of entry is, so indicate that.
15. Next, provide your temporary address in Vietnam. If you’re staying in a hotel, provide the complete address of your hotel.
16. You will also be asked to indicate on the form if you are traveling with a child under 14 years old.
17. Next, indicate what type of visa you are applying for. Do you want a single entry visa or multiple entry visas? Also, indicate how long you will be staying in the country.
18. Finally, indicate if you have any request or other special needs. Then sign the form with your full name.
